Расстройства, вызванные употреблением психоактивных веществ



 

Вопреки некоторым традиционным точкам зрения расстройства, вызванные употреблением психоактивных веществ, являются в основном мотивационными расстройствами и расстройствами действий (см. также Cox & Klinger, 1988; ср. главу 34). Они заключаются в чрезмерной мотивации и чрезмерных действиях, которые чаще всего направлены на прием определенного вещества или на последствия этого приема. При этом мотивами могут служить следующие пункты.

А) Эмоции, индуцированные приемом веществ (например, алкоголь расслабляет и вызывает положительные эмоции).

Б) Процедура приема,которая дает возможность отвлечься от стрессовых ситуаций.

В) Социальное признание как следствие употребления вещества (например, у подростков, если их статус в группе зависит от употребления никотина, алкоголя или других психоактивных веществ).

Г) Улучшенное выполнение поведения (например, более компетентное социальное поведение) за счет редукции торможения и появления позитивных эмоций (с точки зрения теории мотивации здесь речь идет прежде всего о разрешении конфликтов приближения—избегания за счет редукции, обусловленной тревогой мотивации избегания).

Д) Повышение самооценки как следствие положительных эмоций, вызванных употреблением психоактивных веществ, а также как следствие вытекающего из этого социального признания и улучшенного выполнения поведения (см. Silbereisen & Reitzle, 1987).

Е) Избегание абстинентных явлений (если возникла физическая зависимость).

Таким образом, существует большое количество возможных причин стать наркоманом после приема определенных веществ, поэтому неудивительна высокая частота расстройств, вызванных их употреблением. В зависимости от тяжести влечения возникают весьма негативные воздействия на другие мотивационные и поведенческие системы. Они состоят, с одной стороны, в том, что средства для удовлетворения мотивов влечения могут быть получены только за счет эксцессов в других областях (например, криминальности). С другой стороны, они состоят в том, что поведение, обусловленное влечением, и вызывающая его система мотивации по степени зависимости могут привести к существенной, вредящей самому индивиду редукции всех без исключения других мотивационных и поведенческих сфер. Например, лица с тяжелой зависимостью от алкоголя и героина полностью разрушают свою профессиональную и социальную жизнь.
