В предыдущих разделах обобщаются гипотезы и эмпирические данные по поводу факторов, способствующих развитию психических расстройств. Закреплению нарушенного поведения и переживания не обязательно, но часто предшествует нарушение развития в фазах, требующих особенных затрат на адаптацию. Современный уровень исследований со всей очевидностью показывает, что ответ на вопрос, при каких условиях преходящее нарушение развития может привести к развитию персистирующих расстройств, зависит от взаимодействия нескольких групп факторов. Различия в уязвимости — как врожденные, так и приобретенные в ходе истории научения — делают разных людей в разной степени подверженными и восприимчивыми к патогенным влияниям. К этим различиям, возможно, прибавляются еще и половые различия, влияющие на уязвимость и развитие отдельных расстройств. Например, согласно многим исследованиям, мужчинам сложнее преодолеть расставание с партнером, чем женщинам (Ermert, 1996); депрессия же чаще встречается у женщин. Вторая патогенная группа факторов — это стрессовые негативные жизненные события и хронические негативные социальные влияния,которые воздействуют на развитие во всех вариациях — иногда в соответствии с культурным стандартом, иногда выходя за рамки нормы. Сюда относятся не только травматические переживания и патогенные социальные отношения в семье, но и культурные условия, которые либо способствуют некоторым расстройствам, либо смягчают выраженность расстройств. В частности, дискутировался вопрос: может ли быть так, что повышенная болезненность депрессивными расстройствами у женщин и алкоголизмом у мужчин поддерживаются культурно обусловленными половыми стереотипиями и соответствующими влияниями социализации (Widom, 1984).

Однако для развития расстройств и смягчения их выраженности имеют значение не только патогенные факторы — внутренние (в какой-то мере конституциональные) и внешние. Для прогнозирования психических расстройств следует учитывать и протективные факторы (см.главу 9). Здесь тоже можно выделить внутренние и внешние по отношению к индивиду источники. Индивидуальные признаки компетентности в совладании со стрессом (ср. главу 17), интернальный локус контроля, всевозможные профессиональные и социальные способности и особенности социальной поддержки — все это, будучи протективными факторами, способно смягчить патогенные влияния. В частности, к таким протективным факторам относятся стабильные отношения в детстве или в зрелом возрасте, позитивное подкрепление адекватного поведения совладания или социальные модели, позволяющие отвечать на требования жизни адекватно реальности. Сделав обзор несомненных протективных факторов, которые отмечались во многих долговременных и срезовых исследованиях, Эгле, Гофман и Штеффенс (Egle, Hoffmann & Steffens, 1997) обнаружили, что существенным протективным фактором являются позитивные отношения с первично референтными лицами. Сложное взаимодействие патогенных и протективных внутренних и внешних условий — вот от чего зависит главным образом относительная успешность адаптации в процессе развития, который продолжается всю жизнь. Некоторые модели этого взаимодействия представлены в главе 17 (Стресс и копинг) как взаимодействие стрессоров и ресурсов. Факторам, способным уменьшить воздействие патогенных условий, посвящены исследования жизнестойкости (Werner & Smith, 1992), и в будущем в этом направлении необходимо использовать сведения о факторах риска, обусловленных социализацией, что предоставит возможность создать лучшие модели взаимодействия протективных условий и условий риска.
