五、申请材料（均为一式两份）：

1.《中国政府奖学金申请表》（英文填写）；

1.Application Form for Chinese GovernmentScholarship （written in English）;

 

2.经过公证的最高学历证明。如申请人为在校学生，须另外提交本人就读学校出具的在学证明。中英文以外文本须附经公证的中文或英文的译文；

2.Notarized highest diploma;

Prospective diploma recipients must submitofficial document issued by your current school to prove your current studentstatus or expected graduation date. Documents in languages other than Chineseor English must be attached with notarized Chinese or English translations.

 

3.学习成绩单。中英文以外文本须附经公证的中文或英文的译文；

3.Academic transcripts （written in Chinese or English）;

Transcripts in languages other than Chineseor English must be attached with notarized Chinese or English translations.

 

4.来华学习和研究计划。用中文书写（不少于800字）；

4.A Study Plan or Research Proposal （written in Chinese）;This should be a minimum of 800 Chinese Characters.

 

5. 推荐信。两名教授或副教授的推荐信，用中文或英文书写；

5.Two Recommendation Letters（written in Chinese or English）;

Applicants must submit two recommendation letters signedby a professor or an associate professor.

 

6.《外国人体格检查表》复印件。登录国家留学基金委网站http://www.csc.edu.cn/laihua 或http://www.campuschina.org下载，英文填写。申请人应严格按照《外国人体格检查表》中要求的项目进行检查。缺项、未贴本人照片或照片上未盖骑缝章、无医师和医院签字盖章的《外国人体格检查表》无效。检查结果有效期为6个月；

6.Foreigner Physical Examination Form （photocopy）（writtenin English, can be downloaded from http://www.csc.edu.cn/laihua orhttp://www.campuschina.org）;

The physical examinations must cover all ofthe items listed in the Foreigner Physical Examination Form. Incomplete formsor forms without the signature of the attending physician, or the officialstamp of the hospital, or a sealed photograph of the applicant are consideredas invalid. Please carefully plan your physical examination schedule as theresult is valid for only 6 months.

 

7.已获得HSK成绩报告的，请将复印件附在申请材料中。

7.The copy of valid HSK Certificate （if available）；

 

8. 护照照片页、空白页复印件；

Copies of of Passport Photo Pageand one blank Page；

 

以上材料请按上述顺序在左侧装订成册（一式两册）。于2018年3月16日前交至沈阳师范大学国际教育学院办公室（101室）。无论录取与否，申请材料均不返还。

All documents should be bound together onleft in the order of 5.1 to 5.8. （You should submit TWO sets of bound documents）. Submit all your application documents to General Office of InternationalEducation College (Office101)before the deadline March 16，2018 .No application documents will bereturned.
